How, where, and when you cite an image depends on whether you're citing in a Power Point, a web project, a paper, etc. In general, include:

Name of the artist, title of the artwork, date of creation, name of the holding institution & location. Italicize the work's title.

It is very imporatant to attribute images according to your professor's guidelines. Here are more examples from Artstor, an image database to which the library subscribes.
